What should I bring to my initial appointment?

To ensure a productive and efficient initial consultation with Dr Cachalia, please bring the following items with you to your initial appointment:

Referral: If referred by another healthcare provider (GP, Sports Physician or Physiotherapist), please bring the referral letter.
Medicare Card
Private Health Insurance: details (if any) of your current Private Health cover
Up to date medical summary: (current medications, relevant health conditions)
Medication List: A list of all current medications, including prescription, over-the-counter, and supplements.
Recent scans: (Ultrasound, CT, MRI, etc) and blood test results
Questions or Concerns: A list of questions or specific concerns you may have.

Having all these items ready will help Dr Cachalia assess your condition thoroughly and provide the best possible treatment plan tailored to your needs.

As part of your consultation, Dr Cachalia may perform a physical examination. It is important to dress in a way that allows for examination appropriately. We recommend you wear loose, comfortable clothing to your appointment.

Fees & Payments

If you have a valid referral, you may be eligible to claim a Medicare rebate for your consultation. Payment is requested on the day of your consultation and/or procedure. We can process your Medicare rebate directly to your account. We accept EFTPOS, Visa, and Mastercard. Please note that cash payments will not be accepted.

Cancellations

If you need to cancel your appointment, please contact our office during business hours and provide at least 24 hours’ notice so that we can offer your appointment time to another patient on our waiting list.
